Discharge Planning

Discharge planning at Winthrop-University Hospital is handled by Case Managers who assist patients and their families with arrangements for post-hospitalization care. Case Managers are assigned to each patient care unit to assist patients and their families and physicians in developing a discharge plan of care tailored to meet the specific needs of the patient. Once the plan is developed, the Case Manager secures the necessary post-hospitalization services and can provide information about additional community resources when necessary.

Patients who have questions regarding their discharge plans are encouraged to discuss them with their Case Manager who can be reached by dialing extension 2075 from their bedside telephone. Family members may dial (516) 663-2075 from locations outside of the hospital.
