How to Make an Appointment
Winthrop-University Hospital's Cancer Center for Kids is located at 200 Old Country Road, Suite 440, in Mineola, NY. To schedule an appointment:
- Please call: (516) 663-9400
- Information needed when making appointment:
- Patient's first and last name
- Date of Birth
- Health insurance and policy holder
- Home telephone number
- Name of referring physician
- Reason being referred
- Office Hours:
- 8:00 am - 5:00 p.m., Monday - Friday
- Additional late night hours on Thursday, by appointment
- What to Bring to Your First Appointment:
- Insurance card: prescription card
- Referral, if required by insurance
- Blood work or brief medical history
