Two Programs - One Goal CERTIFIED HOME HEALTH AGENCY AND LONG-TERM HOME HEALTH PROGRAM For patients with intermittent skilled healthcare needs, Winthrop's Certified Home Health Agency offers nursing; physical, speech and occupational therapies in conjunction with medical social work and home health aide services. This program is available to residents of Nassau County who have a medical need for at least one of the above skilled services. Coverage for eligible patients is provided through Medicare, Medicaid and many commercial insurance and HMO plans. Special programs include: Joint Replacement, COPD, Diabetes, Congestive Heart Failure, Asthma and Diabetes management, Ostomy, Wound Care, Post Mastectomy, Urogynecology, Pediatrics and Telehealth. CARING COMES HOME We care about you and want you to leave the hospital with confidence. We are especially concerned with how you will manage once you return to your home. The home health services provided by Winthrop-University Hospital's Home Health Agency can help you recuperate in familiar surroundings with the support you need. HOW IT WORKS During hospitalization at Winthrop, a discharge planning nurse will interview the patient and the patient’s family to prepare a safe, appropriate discharge plan. Arrangements can also be made directly by patient’s or family members from home, another hospital or health related facility. A registered nurse from Winthrop's Home Health Agency will coordinate the services you receive, beginning with a consultation with your personal physician and culminating with a plan of care tailored to your individual needs. Your home care coordinator will also report your progress to your physician. INSURANCE COVERAGE - Medicare and Medicaid: For those who qualify for the Home Care benefit, professional services are covered at 100%. - Third Party reimbursement: This varies depending upon your individual policy. Insurance coverage will be investigated by the Home Health Agency. Private payment and sliding scale options are also available. WINTHROP'S HOME CARE PROGRAM - Skilled Nursing: Visits are provided on an intermittent basis when necessary to address your specific needs. - Rehab Services: Registered physical, speech and occupational therapists may be made available to help you work on restoring function. - Medical Social Services: A medical social worker may be made available to help you and your family understand social, emotional and/or personal difficulties related to your health and recovery. - Home Health Aides: If you are receiving skilled nursing or physical therapy, you may qualify to receive home health aide services. Aides will provide assistance with personal care and activities of daily living. This service is for a limited number of days and hours per week and is generally short-term. - Medical Supplies and Equipment: Arrangements may be made for rental or purchase of medical supplies and/or equipment needed for home use. - Intravenous (I.V.) Therapy Services Total Parenteral Nutrition (T.P.N.) Antibiotics Chemotherapy Hydration Pain Management - Telehealth – Daily remote monitoring of vital signs by an R.N., and coordination of necessary services with your physician. Telehealth can monitor blood pressure, heart rate, weight & blood sugar. - SUPPORT AND CARE AT HOME - CLOSE COMMUNICATION WITH YOUR PRIVATE DOCTOR - PERSONALIZED PLAN OF CARE - ASSISTANCE WITH REFERRALS TO OTHER ORGANIZATIONS AND SERVICES IN YOUR COMMUNITY DISEASE MANAGEMENT PROGRAMS Winthrop-University Hospital Home Health Agency also offers special programs in: Asthma Management Program COPD Congestive Heart Failure Diabetes Education & Nutritional Support Joint Replacement Pediatrics Post Mastectomy Post Partum Mood Disorder Urogynecology Wound Care These programs help you to better follow your treatment plan through a personalized teaching program conducted in the comfort of YOUR home. ELIGIBILITY All residents of Nassau County who are under the care of a physician may be eligible regardless of age. TO MAKE YOU FEEL CONFIDENT Winthrop's Home Care hours of operation are 8:00 AM to 4:00 PM, Monday through Saturday. Telephone consultation with a registered nurse is available to patients 24 hours per day, 7 days per week by calling (516) 663-8000; after office hours, call (516) 663-0333 and ask for the On Call Home Care Nurse. * LICENSED BY THE NEW YORK STATE DEPARTMENT OF HEALTH * * CERTIFIED TO PARTICIPATE IN THE MEDICARE, MEDICAID AND BLUE CROSS PROGRAMS * * PARTICIPATES IN MOST MANAGED CARE AND HMO PROGRAMS * * ACCREDITED BY THE JOINT COMMISSION * ****** WINTHROP-UNIVERSITY HOSPITAL LONG-TERM HOME HEALTH PROGRAM Also know as the "Nursing Home Without Walls," the program is open to residents of all ages residing in Nassau Country with a medical problem or disability and who are under the care of a physician. This very special program is designed to offer an alternative to institutional care and coordinate a multitude of medical, social and rehabilitative services for patients who desire to remain in their home setting.
