CARING COMES HOME
We care about you and want you to leave the hospital with confidence. We are especially concerned with how you will manage once you return to your home. The home health services provided by Winthrop-University Hospital's Home Health Agency can help you recuperate in familiar surroundings with the support you need.

HOW IT WORKS
During hospitalization at Winthrop, a discharge planning nurse will interview the patient and the patient’s family to prepare a safe, appropriate discharge plan. Arrangements can also be made directly by patient’s or family members from home, another hospital or health related facility. A registered nurse from Winthrop's Home Health Agency will coordinate the services you receive, beginning with a consultation with your personal physician and culminating with a plan of care tailored to your individual needs. Your home care coordinator will also report your progress to your physician.

INSURANCE COVERAGE
- Medicare and Medicaid: For those who qualify for the Home Care benefit, professional services are covered at 100%.
- Third Party reimbursement: This varies depending upon your individual policy. Insurance coverage will be investigated by the Home Health Agency.
Private payment and sliding scale options are also available.
