The Cardiology senior elective offers the student experiences in physical diagnosis, EEG readings all noninvasive and invasive techniques in patient care. The student will attend catheterization and clinical cardiology conferences, and will join selected surgical conferences. Daily bedside teaching rounds take place as the student is involved as a member of the team in patient care responsibilities. Research opportunities are abundant.

This highly structured elective, which is recommended for four weeks, will provide a core of knowledge of the pathophysiology of disorders of the endocrine system as well as metabolic and nutritional disorders. The student will be exposed to a broad array of endocrine disorders in the outpatient and hospital setting, including diabetes mellitus, thyroid disease, and metabolic bone disease. Students will participate in divisional conferences.

The student will participate in the daily work rounds of the GI Division. This will include the initial evaluation of GI consults and presentation of cases to the teaching attending on daily rounds, at conferences and participation in weekly GI outpatient clinic. Furthermore, the students will learn the various aspects of GI nutrition while participating on GI nutrition rounds. In addition, the student will become familiar with the use of diagnostic and therapeutic endoscopic techniques as well as the proper interpretation of GI X-rays. During the elective the student will have the opportunity to review the pathophysiology of various GI diseases. Participation in GI conferences, including GI case conference, radiology, pathology, and GI Journal Club will be an integral part of the elective.

Geriatric Medicine

Geriatric Subspecialty Resident Education Coordinator

The primary goal is to provide the student with a broad exposure to issues in gerontology and the basis of geriatric medicine. This elective provides a clinical experience in geriatrics through participation in daily rounds, in clinics and consultations as well as attending the Health Update Program for Seniors. A series of core lectures and conferences exposes the student to a range of topics in geriatrics. In addition, the student will be given the opportunity to participate in ongoing research endeavors in the division or develop an independent research project.

Hematology

The student will be provided an overview of benign and malignant hematologic disease. Emphasis will be placed on basic mechanisms of hematologic disease as well as introducing the student to histopathology of bone marrows, peripheral smears and immunocytology. This elective will familiarize the student with the principles of hematology using readings in core areas together with a variety of teaching conferences. Students will also see patients in the clinic as well as consults on the wards of the hospital.

Infectious Disease

The goal is to provide an excellent learning experience in clinical infectious diseases. Upon completion of the elective, the student will be familiar with common infectious disease syndromes. The elective is based on clinical teaching rounds on a daily basis. Core lectures reviews of basic topics in infectious diseases, microbiology, Journal Club, pharmacology and infection control are part of the elective. Slide, tape, and computer assisted instruction programs are also available.

Infectious Disease Research

The research experience in Infectious Disease may be clinical/basic or may involve hospital epidemiology, medical microbiology, mechanisms of virulence, antibiotic effects on bacteria, or clinical projects. A publication incorporating results of the research is prepared in a form suitable for submission to a journal, and the student will be a co-author in the final publication.

Medical Oncology

This elective will familiarize students with the principles of medical oncology. Readings in, and discussion of "core" areas will include carcinogenesis, cytokinetics, biochemistry and pharmacology of anticancer drugs, early cancer diagnosis and screening, staging, and natural history treatment of solid tumors, including autologous stem cell transplantation. Students will see patients in Winthrop-University Hospital Outpatient Oncology Center as well as consults on the oncology and other wards of the hospital. The clinical management of cancer patients will be emphasized in a multi-disciplinary setting with attention to the psychosocial milieu of the patient.

Oncology Research

The student will participate in ongoing research in the Oncology Research Laboratory. Our research is centered in three areas: 1) molecular biology of growth control in melanomas 2) molecular mechanisms of resistance to chemotherapeutic agents and 3) analysis of cell cycle regulated genes. The student will join one of these projects, learn the appropriate procedures and will design and perform experiments. In addition, the student will participate in the regular research conference and present relevant articles from the literature as well as his or her research results.

An individually arranged experience in research is available with prior approval. Students may assist with current research projects and/or pursue approved research activities. Special arrangements are required for this course including appointment of a faculty sponsor. The student will be expected to engage in a significant research activity or to provide significant professional support to a faculty researcher. It is anticipated that the preparation of a written report or paper, suitable for publication, will occur during this experience.

Nephrology

During this elective, the student will be expected to perform the history and physical of a renal patient, stressing the nephrological aspects. The student will be able to plan a work-up for each patient in an orderly fashion and be aware of when to use the various laboratory and radiological procedures in a renal patient. The student will be provided experience regarding the urine sediment examination and be able to know and identify the reversible causes of renal failure, chronic renal failure, acid base abnormalities, interstitial nephritis, hypertension, peritoneal dialysis and hemodialysis, and glomerulopathies. Students will attend weekly conferences and be given assigned readings.

During this elective, the student will become practiced in the neurological examination and history and be able to identify correct findings in a neurological exam. This experience will provide the student the opportunity to synthesize the date in to an anatomical and then pathological diagnosis and to become familiar with the history, diagnosis and therapy of common neurological disorders. The activities of the clerkship include daily patient care rounds, clinic and consultations. There are weekly reading assignments and extensive audiovisual instructional materials available. Weekly conferences are held in neuroradiology, journal club, neuropsychology, outpatient child neurology clinic, and a biweekly neuropathology conference. Students gain outpatient experience in private practitioners' offices.

This course is selective for Stony Brook School of Medicine Students. Available to other students as space permits.

Pulmonary and Critical Care Medicine

This elective is designed to provide the medical student with a comprehensive approach to pulmonary disease. Students will become part of a team including the pulmonary attending, fellow, and housestaff. Daily rounds will be performed on all pulmonary consultations and inpatients on the pulmonary service. Students will learn the approach to the diagnosis and management of diverse pulmonary problems, including pneumonia, COPD, pulmonary embolism, interstitial lung disease, and asthma. Students will perform consultations on interesting patients, which will be reviewed on rounds with the attending. In addition, students will learn to interpret pulmonary function testing and learn the utility of all tests performed in a pulmonary diagnostic lab, including exercise testing and bronchoscopy. In addition, a didactic curriculum, conference schedule, and reading materials are provided in this elective.

This elective is designed to provide the student with comprehensive experience in the diagnosis and management of rheumatologic and immunologic disorders. A diverse outpatient experience at Winthrop and Nassau University Hospital emphasizes management of patients with common chronic rheumatic diseases. Inpatient consultations provide exposure to complex diagnostic and management problems in patients with multisystem disorders. Illustrative cases and management problems are reviewed with faculty during walking rounds. Weekly conference and Journal Club serve to introduce new and controversial topics. Didactic sessions review immunopathologic and biochemical mechanisms of rheumatologic and immunologic disease. Reading and audiovisual materials are provided.

Subinternship in Critical Care Medicine

This course is designed for students who wish to be introduced to the pathophysiology, recognition, and treatment of critically ill patients. The course will be centered in the intensive care unit where a team approach will be used. The student will have the responsibility for critically ill patients under the guidance of a senior resident and full-time critical care staff. Daily rounds with the attending will include clinical pathophysicologic correlations. Chest X-rays will be reviewed daily. A weekly critical care conference will focus on didactic material pertinent to critically ill patients. Core reading material will be provided.

Subinternship/Elective in Medicine

The students are assigned to a medical resident as part of the ward team for the duration of the rotation. Weekly, the student will meet with the Chief Medical Resident and the Vice-Chairman of Medicine to discuss the student's cases. Students participate in Morning Report, Attending Rounds, Chief's Rounds as well as all of the other conferences of Medicine. The subinternship provides "hands-on" experience with varied pathology under the supervision of the full-time faculty. The humanistic aspects of medicine are stressed during the clerkship.

In addition, this rotation provides a special lecture program in preparation for medical internship. Included are special physical diagnosis rounds for the medical students in the areas of Cardiology, Neurology, and General Medicine. Additionally, there are focused lectures in Medical Dermatology. A key component of the course involves Diagnostic Reasoning which is taught by the clerkship preceptor. These unique features are not part of any other elective and are preparation for Medical Internship.

This course is selective for Stony Brook School of Medicine Students. Available to other students as space permits.
