GME : Application Procedures
CHECK-OUT PROCESS FOR OUTGOING WINTHROP HOUSE STAFF
Congratulations on completing your training program at Winthrop! To facilitate the check-out process, please review the pertinent information below on the check-out procedures, and print out and complete the required forms.
1. FINAL PAYCHECKS
Please be advised that your 6/10/11 paycheck (covering period of 5/22/11 through 6/4/11) will be the last check directly deposited.
The final checks (for period 6/5/11 through 6/18/11) and the 2nd check (for period 6/19/11 through 6/30/11) are real checks, and can be picked up in the Academic Affairs Office as of 6/24/11.
Please Note: Academic Affairs cannot release your 2nd final check (period 6/19/11-6/30/11) until you have submitted the Check Out Form, with all required signatures.
If you have fully checked-out of Winthrop, but are unable to pick up your final check, then you must provide the Office of Academic Affairs with an addressed stamped envelope. Final checks mailed out through the OAA may take up to two weeks to be received.

2. CHECK OUT FORM
All outgoing house staff must return this form to the Academic Affairs Office, with all required signatures.
3. FORWARDING ADDRESS FORM
Information contained on this form is needed in order to forward your W-2 Income Tax Form. Please be aware that a current address must be on file. This form is to be returned to the Office of Academic Affairs.
4. NYS EXIT SURVEY
Individuals who are completing a residency or subspecialty residency please fill out the NYS Exit Survey questionnaire via this link:

(NB: those in preliminary years do not complete this survey). Although the survey asks for the respondent’s ACGME residency program number, this is not a required field.
