CHECK-OUT PROCESS FOR OUTGOING WINTHROP HOUSE STAFF

Congratulations on completing your training program at Winthrop! To facilitate the check-out process, please review the pertinent information below on the check-out procedures, and print out and complete the required forms.

1. FINAL PAYCHECKS

Please be advised that your 6/10/09 paycheck (covering period of 5/24/09 through 6/6/09) will be the last check directly deposited.

The final checks (for the pay period 6/7/09 - 6/20/09, and the period 6/20/09 - 6/30/09) are the actual checks, and can be picked up in the Academic Affairs Office during regular business hours (Monday through Friday from 8:30 AM to 5:00 PM) as of 6/24/09.

 

Please Note: Academic Affairs cannot release your final check until you have submitted the Check Out Form, with all required signatures.

If you have checked-out of Winthrop, but are unable to pick up your final check from Academic Affairs, you must provide the OAA with a stamped, self-addressed mailing envelope. Please be aware that final checks mailed out through OAA may take up to two weeks to be received.

 

2. CHECK OUT FORM

All outgoing house staff must return this form to the Academic Affairs Office, with all required signatures.

3. FORWARDING ADDRESS FORM

A forwarding address is needed so that your W-2 Income Tax Form may be mailed to you. This form is to be returned to the Office of Academic Affairs.

4. NYS EXIT SURVEY

Individuals who are completing a residency or subspecialty residency must fill out the NYS Exit Survey questionnaire via this link:

(NB: those in preliminary years do not complete this survey). Although the survey asks for the respondent’s ACGME residency program number, this is not a required field.

When you have completed and submitted the survey, please print out the final page documenting your submission, and bring it to Academic Affairs with your other completed forms.
