CONTINUING MEDICAL EDUCATION CATEGORY ONE CME APPLICATION
INSTRUCTIONS
Winthrop-University Hospital provides on-line access to all CME Category One credit application materials.
These files are in PDF format and require Adobe Acrobat Reader. Once you have downloaded the forms they can be completed electronically by clicking on the form fields and typing in the information.
If you are using either Adobe Acrobat Approval or Adobe Acrobat, the PDF form may be saved with all entered information for later modifications.
If the space allotted in the form is insufficient you may submit additional pages in Microsoft Word format.
The CME Credit Approval Application is to be completed electronically and printed.
The following appendix is also to be completed electronically and printed.

The final CME application packet is to include the CME Credit Approval Application, and the applicable appendices, as summarized in Section VIII of the application form.
