Please complete the Registration Form and bring it to your visit.

For reimbursement by Medicare, the Letter of Medical Necessity must be signed by your doctor. You can bring a signed copy to your visit or ask the doctor's office to fax a signed copy to the Diabetes Education Center at (516) 248-1217.
