Community Health Needs Assessment

A provision of the Affordable Care Act requires that not-for-profit hospitals conduct and publish a Community Health Needs Assessment (CHNA) once every three years and annually report to the IRS the steps it is taking to address the needs it has identified. The following report outlines the assessment process, the findings and a prioritization of significant health needs.
